2. Go to the appointment

Gather your medical records and go to your appointment. Ask any questions you have about medical marijuana.

3. Complete your registration

After your appointment, you will need to complete your medical marijuana registry application. To complete your application, you will need to upload or send copies of two documents that prove your Florida residency of 31 days or more.

Types of Florida Residency

A recent mortgage statement.

Mail from a financial institution from the past two months.

Mail from a government agency from the past two months.

Other documents may be acceptable as well, but they will need to be approved by the Florida Department of Health. You will also need to upload or mail a photo and pay a registration fee.
